The castle on a steep rock above the Freiberger Mulde is first mentioned in 1046 as part of the system of castle districts (Burgwarde) in the frontier marches. In 1084, Emperor Henry IV gave it to Wiprecht von Groitzsch; the Romanesque castle chapel with its checkerboard-frieze portal is among the oldest surviving parts. Frederick Barbarossa received Leisnig as a family estate in 1147 and made the complex an imperial castle in 1158. The gateway and the keep date from this period; the keep's brick upper section is one of the earliest brick structures in Saxony.
Under Margrave William I, Mildenstein was expanded into a prestigious residence in the late 14th century; the front palace, the manor house and the pages' house were built, and the roof truss of the granary loft from 1395/96 survives to this day. The castle never became a main residence, but in the 15th century it served as a nursery for the electoral family, as a widow's seat and, from 1414, as a Wettin administrative seat. In 1706/07 the Polish king Stanisław Leszczyński resided here and signed the Peace of Altranstädt in Leisnig. Later the castle housed a district court and a prison, which was in use until 1953.
From 1890 the Leisnig history and antiquities society displayed its collections at the castle, from 1910 in the Knights' Halls. Today Mildenstein belongs to the Staatliche Schlösser, Burgen und Gärten Sachsen gGmbH. Since the restoration of the manor house in 2017 and the excavation of the 66-meter-deep well, family-friendly exhibitions tell of the castle's architectural history, its administration, the penal system and life at court; a multimedia presentation in the chapel and the Anthro Lab round off the tour.

- Gefangen – Gefoltert – Gerichtet – Using original objects, the exhibition traces how punishment changed from torture to imprisonment – the castle housed a prison from 1504 to 1953.
- Der Hof der jungen Herrschaft – The exhibition in the restored manor house recalls the time when the castle was the nursery of the electoral family, with a hidden-object picture, court costumes to try on and the Prince's Room.

The Romanesque chapel, the exhibition rooms on the first floor (architectural history, penal system) and the large court kitchen are wheelchair-accessible; a companion is recommended. Uneven cobblestones and a 6% incline between the ticket desk and the front palace; door widths 70–99 cm, ramp available for thresholds about 12 cm high. Accessible restroom.
