The foundation of the state collection was laid in 1960 with the purchase of 88 works by Paul Klee from an American private collection, an acquisition pushed forward by the state government under Minister-President Franz Meyers. In 1961, the state of North Rhine-Westphalia then established the Stiftung Kunstsammlung Nordrhein-Westfalen. Its first director, Werner Schmalenbach, systematically built up a holding of major works of classical modernism and American postwar painting until 1990; the collection was initially shown at Schloss Jägerhof.
As space there soon ran short, the state launched an architectural competition in 1975 for a new building on Grabbeplatz, on the site of the former Kunsthalle and the Kunstgewerbemuseum (museum of applied arts). The Danish firm Dissing + Weitling won; the building, with its curved, glossy black granite facade, opened in March 1986 and has shaped the Altstadt (Old Town) opposite the Kunsthalle Düsseldorf ever since.
After a second venue for contemporary art, K21 in the Ständehaus, was added in 2002, the museum on Grabbeplatz took the name K20. From 2008 to July 2010, it was closed for renovation and an extension that added new exhibition halls. Since summer 2024, K20 has presented its collection in a new way: around 200 works tell a global story of 20th-century art, placing women artists of modernism and non-European perspectives on an equal footing with well-known names such as Matisse, Picasso, Mondrian, and Warhol.

According to the museum, generally accessible; ramp to the main entrance, automatic revolving door can be slowed down at the push of a button. Wheelchairs available on loan free of charge at the coat check (advance notice preferred), folding stools as walking aids. Priority admission with a severe disability ID (Schwerbehindertenausweis) when there are lines; companions of blind and visually impaired people free. Guided tours in German Sign Language (DGS; booking via the Stadtverband der Gehörlosen, the city association of the deaf), guided tours and workshops in Leichte Sprache (easy-to-read German), K20 art workshop for adults with cognitive impairments.
